FMSA processes and submits the NGB Form 1/1 as published under Government Gazette No. 26994 (12 November 2004), being the current accepted version of the prescribed form. No further amendments to this form have been gazetted since publication. FMSA submits your completed form to the National Gambling Board on your behalf and confirms submission to you by email. We are not the NGB. We do not control NGB processing timelines, the national register, or enforcement mechanisms after submission.
The onus and responsibility for ensuring that an applicant is removed, banned, and/or excluded from all selected platforms rests entirely with the National Gambling Board and the relevant Provincial Gambling Board(s). FMSA submits your application and follows up on your behalf. FMSA is not liable for any harm, loss, or damage that arises from the failure of any gambling operator, the NGB, or a PGB to enforce your exclusion after your application has been submitted.
